Sutherland, Saskatchewan (parents)Took off from Coningsby for a practice bombing sortie but crashed almost immediately after take-off due to failure of the port inner engine.
Witnesses to the crash stated that the Lancaster climbed slowly to about 200 feet before stalling and falling near the airfield.
