RCAF Station, Rockcliffe, Ontario. Harvard 3106 was in a mid-air collision with Oxford BG-297. Both aircraft crashed five miles west of Richmond, Ontario. Leading Aircraftman Dean was killed in the Harvard, Sergeant S.V. Patterson, Leading Aircraftman F. Rash, and the pilot, Warrant Officer J.R. Shanks, were all killed in the Oxford. Buried in the Elmwood Cemetery (Birmingham), Alabama, USA
