Took off from Grimsby at 23:02 in Wellington Mk IV (Sqn code QT-Z Bomber Command) on an operation to Köln, Germany.

Lost in the North Sea off West Flanders, Belgium.

The bodies of two crewmen washed ashore one near Ostend and one near Zeebrugge several months later. They were buried locally and after the war re-buried at the Canadian Military Cemetery at Adegem, Belgium.
