1918-09-27: Smith died of his injuries in DH-4, F-6077; when it stalled and hit telegraph wires, nose dived to the ground and burst into flames on take off for flight practice. His air-gunner, 2Lt Arthur Leonard Symes (British) was badly shaken, but recovered.
