1919-08-19: MacBeth was killed on a test flight at Castle Bromwich, England of an H P O/400, when the fabric stripped in flight from the lower port bottom wing and the undercarriage collapsed in the forced landing that followed. The other aboard the aircraft were also killed: Lt. E.J. Bravery, (British). 1AM J.B. May, (British), 3AM C.W Offord, (British), 2AM A.J.Winrow, (British), 3AM G. Greenland (British) and 2AM H. Simmonds (British).
