135 Squadron (Pennas Ubique Monstramus). Flight Lieutenant Hart was flying Thunderbolt aircraft HB 977 on a normal training flight when the aircraft suddenly dived into the ground three miles west of the airfield at Minneriye, Ceylon. It is presumed that Flight Lieutenant Hart suffered oxygen failure.
