The OH-58A Kiowa is a 4-place observation helicopter. The Kiowa has two-place pilot seating, although the controls in the left seat are designed to be removed to carry a passenger up front. A total of 74 OH-58A helicopters were delivered to the Canadian Armed Forces between 1971 and 1972 as a Light Observation Helicopter (LOH), performing duties such as reconnaissance, command and liaison or artillery fire direction. The CAF re-designated them as the CH-136 Kiowa. Although similar in appearance to the trainer Bell CH-139 Jet Ranger, major differences between the two helicopters include the main rotor blades and dynamic components. The crew set up for a circuit and took off. On final approach just before touchdown, the a/c developed a yaw, contacted the water and rolled inverted. Both pilots egressed under water and climbed on top of the inverted helo. The crew escaped with minor injuries. Ex US Army OH-58A-BF 71-20900. With 444 Squadron at CFB Lahr, Germany. Destroyed when it struck wires during low flight near Avasinis, north east Italy on 16 May 1976. the flight had originally departed Venzone, Italy on a reconnaissance mission in an earthquake affected area to familiarize the crew with the area while looking for other damage. The crew observed some problems with the dam in the area just north of Avasinis and proceeded up the valley towards the dam for further inspection. Exiting the valley, the pilot flew into an undetected cable. The helo was flying at approximately 200 ft above the ground at 60 knots. The cable slid up the windshield, cut through the upper cowling and then contacted the mast and control rods. The cable snapped and became entangled in the mast. The pilot initiated a descent but approximately 50 ft above ground, the control rods severed and the main rotor decelerated. The pilot lost control as a result and the helo yawed left, hit the ground nose low and rolled inverted. Captain R.G. "Buck" McBride was killed in the crash and the rest of the crew was injured. This accident contributed directly to further CF interest in the Bristol Canada Wire Strike Protection System (WSPS).Known Squadron Assignments: 444

Ex US Army OH-58A-BF 71-20905. Destroyed in a crash on 5 February 1993, north of Fredericton, NB. during a flight from Gagetown. The crew (a pilot, observer and a passenger) encountered deteriorating weather with heavy snow showers. The pilot commenced a low level turn to avoid a snow shower but the a/c was enveloped and spatial disorientation ensued. The a/c contacted the trees and suffered Category "A" damage. Ex-US Army OH-58A-BF 71-20907. Initially delivered to 3 CFTTS at Portage, MB. This a/c was serving with 408 (Tac Hel) Sqn at CFB Edmonton, AB when it was destroyed in a crash at Nakusp, BC on 7 July 1988. A formation of two Kiowa helicopters was proceeding from CFB Edmonton to McDonald Lake Provincial Park south Nakusp, BC to take part in an annual mountain flying training exercise. The formation entered the Arrow Lakes region from the north and proceeded southward in a loose trail formation flying low over the lake 200-300 metres offshore. A climb was initiated to 200-300 ft AGL to fly around the east side of the town of Nakusp after which the formation headed back out over the lake commencing a shallow descent. A motorboat was overflown at low altitude and high speed. Approximately 1 kilometre past the boat, the lead helicopter impacted the water and sank in approximately 180 feet of water. Lt. J.B. Gregoire and another student pilot were both killed in the crash.Known Squadron Assignments: 403;408

Ex US Army OH-58A-BF 71-20923. Category A damage in a crash at Findlay Lake, Quebec on 12 or 13 June 1985. The helicopter was on a night navigation exercise, when three distress radio calls totalling 12 seconds in duration were heard alluding to an undetermined control problem. The helo crashed into the lake. With 427 (Tac Hel) Sqn at CFB Petawawa, ON at that time. Major R.E. Connell and Sgt. H.A. Anderson were both killed in the crash. After an extensive technical investigation, the cause of the accident was traced to an a/c component (free-wheeling unit) failure that effectively disconnected the engine from the drive train but with no indication to the crew of the problem.Known Squadron Assignments: 403;430;427

Ex US Army OH-58A-BF 71-20933. To 408 (Tac Hel) Squadron, CFB Namao, Alberta on 16 October 1972, part of last batch of 4 Kiowas to this unit. Category A damage in a crash at CFB Suffield, Alberta on 16 November 1984, still with 408 Squadron. The helo was on an authorized night training mission within the confines of the CFB Suffield training area. The mission profile included a night navigation segment combined with a night field landing exercise. This field landing exercise consisted of landings and take-offs assisted by combat vehicle lights. The terrain was composed of rolling hills with very smooth contours and the total snow cover provided very little contrast. While conducting its third approach the helo was seen to begin a rapid descent shortly after turning base leg. The helicopter struck the ground inverted at high speed and was completely destroyed. Lt. R. Stam and Flight Sergeant W.P. Black were both killed in the crash. Despite a detailed investigations of the flight controls and dynamic components, no traceable technical fault could be found for the crash.Known Squadron Assignments: AETE;408;403;430

Ex US Army OH-58A-BF 71-20935. With 403 (HOT) Squadron at CFB Gagetown, NB. On 16 September 1986, while on final during an Air Mobile (Exercise BRAVE LION) operation in Norway, the a/c struck a wire and crashed. The initial wire contact was in the rotor mast area causing an immediate loss of control. The WSPS had not engaged the war due the shallow closing angle. Both crew members survived the crash and airframe suffered Cat "B" damage. With 430 ETH at CFB Valcartier, QC as of February 1995.
