1918-02-10: Fachnie was injured in Avro 504, D-12; when he allowed the fuel tank pressure to become too low causing engine failure and then misjudged the forced landing in a field. 1918-10-02: Fachnie was injured in Camel, E-7297; when coming down to land from 100 feet attempted to turn with insufficient bank, got into spin and continued to spin into ground.
