1915-12-30: confirmed as Flt sub-Lieut. 1916-05-28: RAec cert. #2975 1919-02-03: confirmed staff officer, Captain 1946-09-02: Stephens died in The Hospital, Hurst Rd, Horsham, Sussex, England, this date. His death was due to the effects the the wounds/injuries suffered on 1917-06-13, when he returned from a bombing raid in his Strutter, with a hung bomb under the aircraft. In attempting to stabilize it, it dropped and exploded.
