1918-02-18: Fachnie was injured at 90RS, Shotwick, in Avro 504, D-12; when he allowed the fuel tank pressure to drop too low, causing engine failure. He misjudged his forced landing in a field and crashed. 1918-10-02: Fachnie was injured in Camel, E-7297; at 51TDS, when coming down to land from 100 feet, attempted to turn with insufficient bank, got into a spin and continued to spin into the ground.
